Dear MTA Members: Attached you will see a form for “pre-designation” of a Workman’s Compensation Physician. You have the right to pre-designate your own physician as your Workman’s Compensation Physician. This form is provided by CTA.In the event that you become injured on the job, the District will assign you a physician through Keenan and Associates as your Workman’s Compensation Physician. This could take a lot of extra time and result in you being examined by a physician who has absolutely no previous knowledge of your physical condition. (You have the right to file this pre-designation, but it is not a requirement for a Workman’s Compensation claim) In order to file this form:
- You will need to use the attached form to file your pre-designation with the District Office. Your Physician must sign the form in order to accept the responsibility of being your Workman’s Compensation Physician. If he/she does not sign it, it won’t work.
- You must file the completed form with the District Office before you file a Workman’s Compensation Claim.
